Forward Head Posture: How To Fix It, Starting With The Upper Back And Legs

Forward head posture: the short answer

Forward head posture means your head sits ahead of your shoulders instead of over them. You fix it from underneath, not by pulling the chin back. Strong legs give you a steady base, the upper back and shoulder blades hold the chest open, and then the head can rest over the body. In Iyengar yoga the points are: shoulders back and down, shoulder blades moving in towards the chest, and the chin level, not poking forward. Here is the article in short form:

  • Forward head posture is the head ahead of the shoulders, whatever the cause.
  • Long hours at a desk, driving with drooping shoulders and piles of soft pillows all add to it.
  • Reviews of exercise found shoulder blade and deep neck exercises moved the head back, though the evidence is still building.
  • Geeta Iyengar: without strong legs and feet, the head cannot be held in correct alignment with the spine.
  • Chin level, shoulders back and down, blades in. Open the armpits.
  • Do not force the chin back. Go a little at a time.
  • In Darwin, learn it in a beginner class or online.

A quick word on Iyengar yoga. Iyengar yoga is the style developed by Mr BKS Iyengar. In the 20th century he did more than anyone to set down traditional Indian yoga, through his books and his teaching, and he pioneered the use of props. It is the style we teach at Flametree, in Darwin and live online.

What is forward head posture and what causes it?

Forward head posture is the head sitting forward of the line of the shoulders at rest. Researchers often measure it with the craniovertebral angle (the angle between a line from the base of the neck to the ear and a flat line across). A smaller angle means the head is further forward.

Where does your head sit? In a tall, balanced posture the ear is over the shoulder. In forward head posture it sits ahead.

The causes are everyday ones. The Yoga Rahasya writers, who wrote on yoga and the body, name long hours at the desk, driving with drooping shoulders and sleeping on piles of soft pillows. Most of us also stand more on one foot, sit more on one buttock and let the lower back sink when we sit. We do not notice any of this until something hurts.

If your habit is mostly phones and screens, see our page on tech neck. This page is about the head position itself, whatever put it there.

How do you fix forward head posture?

Begin below the neck. Geeta Iyengar wrote: “Without the firm foundation of strong legs and feet, the brain, which is the seat of intelligence, cannot be held in correct alignment with the spine. Hence the standing poses are introduced first.” That is why our beginner classes start with standing poses. The head cannot sit properly over a body that is collapsed underneath it.

Then work the three points Geeta and the Yoga Rahasya writers come back to again and again:

  1. Shoulders back and down.
  2. Shoulder blades move in towards the chest.
  3. Chin level, not poking forward. Keep the neck soft and the teeth and jaw unclenched.

Her line to keep in mind: “Open the armpits and you are safeguarding the neck.” Mr Iyengar also warned against clenching: “Do not clench your teeth or you will also be ‘clenching’ your brain.”

Do not yank the chin back to look straight. That only tightens the neck. Let the head come back over the body as the legs, chest and upper back do their work.

What do the exercise reviews say?

Three recent reviews looked at exercise for forward head posture, and they point the same way,, with limits on how sure they are.

  • Shoulder blade exercises. A 2024 review of 8 randomised trials (trials where people are put into groups by chance) with 313 people found that treatment aimed at the shoulder blades (the scapulae) reduced chronic neck pain, and the forward head posture appeared corrected (Chen and colleagues, BMC Musculoskeletal Disorders).
  • Deep neck exercises. A 2026 review of 25 trials with 1,166 people found craniocervical flexor exercise (training the deep muscles at the front of the neck) gave a moderate improvement in the craniovertebral angle. The authors called the findings suggestive rather than definitive (Javdaneh and colleagues, Pain Management).
  • Breathing. A 2025 review found breathing interventions gave a moderate improvement in the craniovertebral angle, though the certainty of the evidence was very low (Park, Kim and Kang, Bioengineering).

An Iyengar class does all three in one session: the shoulder blades work in the standing poses, the chest opens, and the breath is trained. These trials did not test yoga classes, so we do not claim they did. What they show is that the parts of our practice that move the head back are the parts that work.

Which yoga poses help forward head posture?

  • Mountain Pose (Tadasana), with a block between the thighs. The block wakes the inner legs. Lift the chest, take the shoulders back and down and keep the chin level.
  • Warrior 2 (Virabhadrasana 2). Strong legs, arms out level. The arms and shoulder blades work hard and the chest opens.
  • Kneeling with the arms overhead. Kneel and stretch both arms straight up. The armpits and chest open and the upper back lengthens.
  • Triangle Pose at a wall or pillar. This is what Mr Iyengar gave Christina, the woman with the rounded upper back, with a brick along her upper back.

Mr Iyengar found his own alignment against a wall. Geeta told the story at a props demonstration in Sydney in 1996: “The wall is my Guru.” Stand with your back to a wall to check where your head sits. Do not strain to reach it. Just notice, then come back to it a little at a time.

Can yoga correct forward head posture?

Yoga works on the causes. In a 2009 Indian trial, 291 professional computer users were randomly split in two. One group did an hour of yoga a day, five days a week, for 60 days. The other group was put on a wait-list (Telles and colleagues, Work). Of those, 118 finished and were analysed. The yoga group’s aches and pains came less often, hurt less and got in the way less. The wait-list group’s aches got worse. It did not measure the head position, so it is evidence for the desk worker’s comfort, not for the angle.

A 2019 review of 10 trials (686 people) found yoga eased long-lasting neck pain and freed up neck movement. The posture of the whole back matters too: for the rounded upper back, our guide to yoga for posture covers the trial on thin bones and curved spines. For the neck itself see stiff neck, yoga for neck pain and upper back pain.
